How does the grant work?
The main purpose of the grant is to make it easier to get needed dental care by decreasing the cost for the patients. This is done in several ways.
First, Dr. Bo Crofoot with Corner Stone Family Dental is donating x-rays, exam fees, and his time to decrease the cost to those who qualify. He is also donating some extra treatment not reimbursed by the grant. Therefore it is necessary that all work done for the grant be done at Corner Stone Family Dental in Sugar City.
Second, Sugar City is paying part of the dental bill with grant funds from The State of Idaho. For those who fit the qualifications of the grant, a screening appointment will be given and necessary x-rays taken. Those will be reveiwed, and patients showing the most need will be given priority. Selected individuals will be notified of which procedure(s) they may receive and their expected contribution. An appointment will be set. When the work is completed a report is given to the city, and the city will pay its portion. Payments may be arranged for the patient's portion.
Due to limited funds, not everyone who meets the standards can receive help, but we will do our best to help as many people as possible. It is also very important to keep appointments or reschedule with a minimum of 24 hours notice. Patients who do not keep their appointment may be removed from the program even after being accepted.
An example:
Bill broke his tooth, but he doesn't have dental insurance and his budget is a little tight for a visit to the dentist. He fits all the criteria for help. He fills out the application takes it into Corner Stone Family Dental, makes an appointment and has his screening appointment. He has xrays and an exam. He is accepted for help, and has his tooth filled. He makes arrangements for payments and goes home. Corner Stone Family Dental informs Sugar City that Bill had a filling on such a date and the city pays part of the bill.

Sugar City uses grant funds:
Grant money $ 38
Bill's saved $177
Bill's final cost $114
